Admissions
To assure that necessary information and documents are obtained for the admission process, the resident or their responsible party should bring the following items to Westhaven Community prior to or on the day of admission:
- Social Security Card
- Medicare Card
- Insurance Card(s)
- Medicaid Card (if applicable)
- Guardian/Conservatory Papers
- Power of Attorney
- Advanced Directives (Living Will and/or Durable Powere of Attorney for Healthcare)
