Call Social Services at (515) 432-1392 and request that a packet be sent to you, or download the application in the Admissions area on the website.
What payment methods are accepted?
Westhaven Community accepts Medicare, Medicaid, private insurance and private pay.
How do you get on the waiting list?
Fill out an application and return it to the Social Services Department. Then, meet with Social Services to explain your need.
